I release Soul Haum Yoga and its owners, employees, and agents, and hold them from any and all liability arising out of any personal injuries or damages, foreseeable or unforeseeable, which may occur as a result of my participation in any class, program or activity sponsored by Soul Haum Yoga. I hereby declare myself physically and mentally sound and capable of participation in those activities, programs and classes.

I permit the use of class photos taken to be used for Soul Haum marketing.

I understand that there will be no refund in the case of a no show. I understand that if I arrive more than 10 minutes late to class, I will not be permitted to join the class with no refund. I understand that I must contact Soul Haum via phone or email to notify of a late cancellation (less than 12 hours before class) in order to maintain the class credit on my account.

I agree to the Waiver of Liability and Photo Release.
